Complete the application form and submit to Fox Valley Technical College (FVTC) no
later than Friday, October 17, 2008. For assistance in obtaining an application form, contact FVTC at (888)
370-1752 or cjgrants@fvtc.edu between the hours of 7:00 am and 5:00 pm
(CST) Monday – Friday.
The application fee is $400 per person. The registration fee does not include travel,
lodging or meal expenses. The participant is responsible for these
costs. A limited number of full and partial scholarships are available to past NIYPA graduates. Learn more about scholarships for the NIYPA Leadership Council.
The following materials must accompany your admission request:
- completed application form
- a copy of a recent photo ID (for verification)
- payment of $400 OR, if applying for a scholarship
- responses to the scholarship questions
Materials must be received by
Fox Valley Technical College no later than Friday,
