Federal Direct Loan Additional Loan Request *2012-2013 Fall/Spring*

To initially apply for a Federal Direct Loan, you must complete the Free Application for Federal Student Aid (FAFSA) and complete the financial aid application process.

This form is to be used by INDEPENDENT students who already have a Federal Direct Loan awarded to them and are requesting an additional loan amount. If you are unsure of your eligibility, please visit www.fvtc.edu/finaid/studentloans.

The amount requested is in addition to loans already offered. Write only the amount of increase you are requesting. If you request more than your eligibility allows, the loan will be certified to your maximum eligibility.
