Each year, this distinguished honor is presented to a deserving alumnus who has completed a Fox Valley Technical College associate degree, technical diploma or apprentice program. Nominees must have achieved a high level within their chosen field of endeavor, which is exemplified by having attained increasingly responsible positions in their company, organization or community.
Applications are due by 12 noon (CST) on March 3, 2014. The FVTC Outstanding Alumni Award recipient will be selected by committee, and recognized at the spring commencement ceremony on May 18, 2014. This individual will then be considered as FVTC's nominee for the Wisconsin Technical College District Board Association's state-wide "Distinguished Alumni Award" in August 2014.
If you have submitted a nomination in the past and that candidate was not selected as the recipient, we welcome you to submit a new entry for that candidate.
Criteria for Nominations:
- The nominee must have completed a Fox Valley Technical College associate degree, technical diploma or apprentice program (verified by the College).
- The nominee may be a current employee of Fox Valley Technical College.
- Nominations may be submitted by employers, colleagues, friends, family or by the alumnus.

AWARD NOMINATION CRITERIA: "Letter of Support" is to be provided by nominator, or other outside source. Questions 1-5 are to be completed by the nominee/alumnus. Items may be submitted together or separately as long as all indentifying information is complete.
Letter of Support (written by nominator): Please provide a letter of support drafted by the nominator or other reference source (not the nominee/alumnus). Letter should be 150 words or less and should explain why the nominee deserves the Outstanding Alumni of the Year Award specific to the criteria outlined within this nomination form.
The Letter of Support should also include all of the following information:
- Author's Name

- Author's Phone Number
