In October, 1998 the state of Wisconsin initiated “a check of the backgrounds of persons who provide care for others or who have access to people who receive care.”
One of the prerequisites you have to complete before you can enter the core nursing classes includes that you submit the results of a “Wisconsin Criminal History Single name Record Request” (Criminal Background check). When you send for this document you will also need to request a Caregiver Background Check by DHFS (Department of Health and Family Services). You can access the form to obtain these reports at http://www.doj.state.wi.us/dles/cib/forms/cib/djle250.pdf or from the Counseling office in E100).
You should keep the original document and submit photocopies of the reports to Sue Popp (C-102) once you have enrolled in Semester 1 core classes. Please keep these documents in a safe place so that you can retrieve them whenever needed
If you have lived outside of Wisconsin less than three years, you must also request a criminal background check from the state or states in which you did reside during this three year time frame. You can obtain a list of the contact agencies for other states from the counselor office or access this list online at http://www.casanet.org/program-management/volunteer-manage/criminal-bkg-check.htm.
