ADMINISTRATIVE POLICY TYPE: STUDENTS

POLICY TITLE: Liability for Student Life Activities

Any student or staff member participating in a school sponsored activity must sign up in the Student Life Center and provide the required information. All participants are required to sign a Waiver of Liability form. This form releases Fox Valley Technical College from any liability for personal injury, property loss or damage resulting from the event.

All those who participate in intramural sports are required to complete the Intramural Sign-Up form which includes an insurance waiver releasing FVTC from liability in connection with the sport.

All those who participate in club sports are required to complete a Release of Waiver of Liability and Hold Harmless Agreement form, provide proof of insurance, complete a physical and maintain good academic standing.

All forms are available in the Student Life Center.
