What Do I Need to Purchase?

Background Check + Health Portal

Required for all Health Division students

Cost: $73 (non-refundable)

You may not need to purchase a new background check if all of the following are true:

  • Your previous FVTC Health Division background check was completed within the last four years.
  • The background check will remain valid throughout the current academic year.
  • You have not had any criminal charges since the background check was completed.
  • You have not legally changed your name.

If you have a Social Security Number, you will need to provide it to purchase your Background Check. Please have number handy.

 

Drug Screen

Additional Cost: $46 (non-refundable)

Drug screen requirements vary by program.

Drug Screen Required

  • Associate Degree Nursing (ADN)
  • ADN Part-Time
  • Medical Assistant
  • Medical Laboratory Technician
  • Neurodiagnostic Technology
  • Practical Nursing
  • Substance Use Disorder Counseling
  • Surgical Technology

Drug Screen Required Only if Requested

  • Health Information Technology
  • Nurse Refresher
  • Occupational Therapy Assistant

Drug Screen Not Required

  • Dental Assisting
  • Dental Hygiene

Important: If your program requires a drug screen, additional instructions will be provided as part of your compliance requirements.

Viewpoint Step-By-Step Instructions

Step 1: Get Started
  1. Go to https://www.viewpointscreening.com/fvtc
  2. Select 'Start Your Order'
  3. Choose your department
  4. Package Summary: review your package and acknowledge the terms of use policy
Step 2: Complete the Background Information Disclosure (BID) Form

The State of Wisconsin requires all students to complete a Background Information Disclosure (BID) form.

When completing the form:

  • Answer all questions completely and accurately.
  • If reporting previous addresses, include the county and state when possible.
  • If reporting criminal history, provide complete information to avoid delays.

Step 3: Complete Payment

After submitting the BID form, you will return to the application screen.

Review your information and complete the payment process.

Step 4: Log In to Viewpoint

After your order is complete:

  • Create or update your password.
  • Log out of your Viewpoint account.
  • Log back in to your Viewpoint account.

Your username will be the email address you used when creating your account.

Step 5: Open the Health Portal + Review Requirements

From your dashboard, click Health Portal.

This is where you will:

  • View your requirements
  • Read instructions
  • Download forms
  • Upload documents
  • Track approval status

Review Requirement Instructions:

Click on each requirement to read the description and instructions.

The requirement description will explain:

  • What documentation is needed
  • Which forms are accepted
  • How to complete the requirement
  • Important submission details

Be sure to read the full instructions before uploading documents.

Step 6: Upload Your Documents

Upload the required documents for each requirement.

Before uploading, make sure:

  • Your name is visible on the document.
  • The document is complete and easy to read.
  • You are uploading the correct file.
Step 7: Monitor Your Account

Continue checking your Viewpoint account until all requirements show Approved.

You may receive weekly reminder emails until all documentation has been approved.

Important: Uploading a document does not mean the requirement is complete. A requirement is complete only after the document has been reviewed and approved.
